2009 Annual Convention - General Convention Information & Registration Fees

CONVENTION REGISTRATION INCLUDES: Educational Sessions, Admission to Exhibit Hall, Wine & Cheese Welcome Reception (Thurs.), Exhibit Hall Breaks, and Morning Hot Beverages.

The early bird Fees end on Friday, March 20, 2009 and after this date regular fees apply. There is an additional $20.00 fee for on-site registration.

Tax Deduction for Education US Treasury regulation 1.162-5 permits an income tax deduction for education expenses (registration fees and cost of travel, meals and lodging) undertaken to: 1) maintain or improve skills required for one’s employment or other trade or business, or 2) to meet expressed requirements of an employer or a law imposed as a condition for retention of employment, job status or rate of compensation.



REGISTRATION
NJSHA strongly encourages you to pre-register. You can save $20 - $40, avoid lines at onsite registration, and guarantee yourself a seat and handouts at sessions - so register today!

THREE ways to register.
  1. Online at the NJSHA website: www.njsha.org. Even if you are paying with a check or PO, register online and just mail in payment.
  2. Fax in your registration to (908) 450-1119. Even if you are paying with a check or PO, fax in your registration.
  3. Mail in your registration to: NJSHA, 66 Witherspoon Street, Suite 337, Princeton, NJ 08542
No registration forms should be mailed after Friday, April 24, 2009 Online and faxed registrations will be accepted up until Wednesday, April 29, 2009. However, after the 29th registration must be done on-site. An additional $20 will be charged for on-site registration, so please remember to send in your forms early and save!



PAYMENT
Registration and dues payments can be made by VISA, Mastercard, American Express, check (made payable to NJSHA), money order or purchase order. A $20 service charge will be assessed for all returned checks.

STUDENTS
To qualify for student registration rates you must be a NJSHA Student Member. To qualify as a Student Member you must provide a signed letter from your department chair or graduate advisor indicating that you are enrolled in a program taking a minimum of 9 credits for graduate or 12 credits for undergraduate students per semester along with your application that is included in this brochure.

COURTESY RATES
NJSHA extends Member Rates to members of the New York Speech-Language-Hearing Association, Pennsylvania Speech-Language-Hearing Association and Delaware Speech-Language-Hearing Association. Members of state Occupational Therapy Associations and Physical Therapy Associations may also attend at member rates provided they are members of their state professional associations. Proof of membership is required with your registration materials.



CONFIRMATION
Email confirmations will be sent weekly. Then approximately two weeks prior to the convention you will also be mailed a confirmation packet, which will include the following; a confirmation notice, your name badge and holder, your Awards Reception tickets (if applicable), directions, and additional information. Please remember to bring your name badge with you. A $3.00 fee will be charged for replacing lost or forgotten name badges. Also, please bring this registration booklet with you; abstracts will not be included in the convention program book.



REFUNDS
All cancellation requests must be submitted in writing by Friday, April 24th at 4:00 pm. You may email your cancellation request to i...@njsha.org or fax to (908) 450-1119. Cancellations will not be taken by phone. A $20 cancellation fee will be deducted to cover administrative expenses. All refunds will be processed after the Convention.
